Comercialización de los insumos ofertados en In	La evaluación de este criterio se realizará a partir de la información entregada por el proponente conforme a lo indicado en su oferta técnica (Anexo Nº6) y de acuerdo a la siguiente fórmula: (Q de certificados presentados correctamente *100)/ Mayor Q de certificados presentados correctamente. Q = Cantidad	20%
2 d) precio	Se asignará 100 puntos a la oferta de menor valor, para la cual se determinará el precio promedio proporcional, el que se obtendrá de acuerdo a las siguientes operaciones aritméticas. El siguiente criterio tienen por finalidad obtener un precio promedio proporcional de acuerdo al consumo histórico obtenido directamente del consumo de los últimos 12 meses en relación a lo informado por el Servicio de Oftalmología y bodega de Insumos Clínicos. Los porcentajes asignados para cada lente serán los siguientes: -El precio ofertado por el Lente Intraocular acrílico hidrofóbico monofocal (Línea 1), será multiplicado por un factor porcentual de 0.3. -El precio ofertado por el Lente intraocular monofocal monopieza esférico (Línea 2), será multiplicado por un factor porcentual de 0.3. -El precio ofertado por el Lente intraocular de respaldo múltipieza (Línea 3), será multiplicado por un factor porcentual de 0.2. -El precio ofertado por el Lente intraocular de respaldo de cámara anterior (Línea 4),	40%
3 Cumplimiento de requisitos formales	a) Cumplimiento de requisitos formales: Este criterio de evaluación corresponde al cumplimiento de los oferentes en la oportuna, completa y correcta presentación de sus propuestas en términos formales respecto de toda la documentación exigida en el artículo 13o. La evaluación de este criterio se efectuará de acuerdo a lo siguiente: Cumplimiento de requisitos formales Puntaje Oferente cumple con todos los requisitos formales de presentación de la propuesta al momento de su presentación. 100 Oferente cumple con todos los requisitos formales de presentación de la propuesta después de haber sido requerido por el HOSPITAL de conformidad con lo previsto en la letra f) del artículo 12° de estas Bases o se Ie haya solicitado salvar errores u omisiones formales en conformidad al artículo 16° de estas Bases. 30 Oferente no cumple con todos los requisitos formales de la propuesta, habiendo omitido antecedentes o certificaciones al momento de presentar su oferta no obstante haber sido requ	5%
